Functional Assessment
At Adapted Mind, we provide comprehensive Functional Assessments to evaluate how an individual manages the practical demands of daily life. These assessments are essential for understanding strengths and challenges across key areas of functioning and are commonly used to support access to services such as NDIS funding, school-based supports, or transition planning.
Led by a Clinical Psychologist, our functional assessments explore the individual’s capacity to perform age-appropriate tasks and participate meaningfully in everyday activities at home, school, work, and in the community.
What Does a Functional Assessment Involve?
Functional Assessment typically includes the following components:
- Clinical Interview: A detailed conversation with the individual (and /or parent/caregiver) to explore areas of concern, developmental history, daily routines, and the level of independence in different life domains.
- Standardised Assessment Tools: We use validated tools such as the Adaptive Behaviour Assessment System, Third Edition (ABAS-3) to assess functioning across domains, including:
- Communication
- Self-care and daily living skills
- Social and interpersonal functioning
- Functional academic skills
- Community use and safety
- Health and leisure activities
- Work or vocational readiness (for older adolescents and adults)
- Multi-Informant Input: Where appropriate, input is gathered from parents, teachers, support workers, or allied health professionals to ensure a comprehensive picture of functioning across environments.
Why Is a Functional Assessment Helpful?

- NDIS Applications or Reviews: Supporting evidence for access to NDIS or to determine the appropriate level of support funding.
- School or Educational Planning: Informing learning support plans, transition strategies, or school funding applications.
- Diagnosis Support: Providing essential information when assessing conditions such as Intellectual Disability, Autism Spectrum Disorder, or global developmental delays.
- Life Skills Planning: Identifying goals and supports needed for developing independence in adolescence or adulthood.

- Following the assessment, you will receive a comprehensive report that includes:
- Standardised scores and interpretations
- Strengths-based insights and areas for support
- Clear, practical recommendations for home, school, or care settings
A feedback session is provided to discuss the results, answer questions, and ensure you feel confident about the next steps.
